First I have to thank Dan for the wonderful answers he's providing on this list, and the service of maintaining and improving swfmill. I think you're amazing!
I don't know if this will be worth anything but thought I'd through it out as another possible cause.
We have server code that issues swfmill transforms (swf2xml, replace stuff in xml, then xml2swf kind of thing). When we ran it in windows it was working but when we ran it in linux it was failing silently if there were what linux considers special shell characters in the glyphs (like $ ). If I remember correctly, for us what worked was that our linux admin recompiled swfmill making sure to statically link in all the needed libraries, whereas when it was failing we were using a version he'd compiled that relied on dynamically linking libraries.
