Yes, using an flv with cue points for the "star" displaying is the key and then using somesort of start hit detection and a check with the user's input is the other part.

I'm actually not worried about doing the game, i'm more worried about buying a control i won't ever use. LOL

Well, I don't know about that particular guitar, but it should connect to the Wiimote and activate the wiimote's buttons, and the Wiimote then transmits back to the PC/Wii console.  

With Wiiflash of course, you don't need a Wii console, just a Wiimote, Bluetooth dongle, PC, and WiiFlash software and classes.  So you should be able to do this with all that plus a Wii guitar.  Programming a game to work with music would be the hard part, but you could use cue points in an FLV of the song (no video, just the song) to know if the user strummed at approximately the right time.
